Six patients with complex carotid-subclavian artery anastomoses were surgically treated at our institution between 1979 and 1989. The anastomoses involved the distal carotid artery (four patients), or carotid bifurcation (two patients), or common carotid artery (one patient), with an intimal flap (four patients), and were associated with aortic atheroma (three patients) or atherosclerotic aneurysm (three patients). The main indications for treatment were: combined carotid-subclavian angiography in four patients, or vertebral artery thrombosis in one patient; recurrent vertebrobasilar insufficiency in four patients; and subclavian steal syndrome in one patient. Three patients had elective surgery; two patients had urgent surgery; and one patient had a previously untreated subclavian steal syndrome. There were no operative deaths; the single postoperative death was attributed to a postoperative ischemic stroke. All five patients who underwent carotid endarterectomy (three elective, two urgent) survived. Postoperative angiography showed excellent results in all patients.
