Fall prevention interventions include training to improve balance and fall risk [ 94 95 ]. Specific fall prevention training for older adults is focused on improving strength, proprioception, and cognitive functioning. Strength training has been proven to be effective in increasing lower extremity muscle strength [ 96 97 ]. Proprioceptive training addresses the nervous system’s ability to process and respond to information from the proprioceptors. Proprioception involves the two primary types of proprioceptive receptors, the muscle spindles and the joints receptors [ 97 ]. These proprioceptive receptors detect acceleration and deceleration of the joints, muscle tension changes, bending of the body, limb-to-body contact and limb tension [ 97 ].

The challenge of this new technology is to improve game play in the development of the ideal balance game and to identify the target population that can benefit from its use. Currently there are not many studies that focus on the most important factor in exergaming development: the design of the game. The studies reviewed in this paper are missing or evaluate several of the most important features of an exergaming system. They do not evaluate if an exergaming system actually leads to improved balance abilities. Furthermore the variability in the systems used among the studies makes it difficult to compare the results among them.

The development of an exergaming system for balance training needs to be carefully thought of. The first step is the selection of the outcome measure [ 79 ]. A strength of the studies reviewed in this paper is that they select an outcome measure that directly relates to balance, thus making direct comparisons of the results among them possible.

Two studies measured COP displacement, given by the difference in pressure between 2 points of interest [ 23, 25 ]. The COP displacement as measured during gameplay is thought to provide information on proprioception and low-level motor control. Seemingly contradictory findings exist in the literature regarding the level of COP displacement generated by subjects performing various balance tasks [ 25, 33, 42 ]. To date it is unclear to which extent the COP displacement during gameplay measures basic postural control, gait related movements or also afferent sensory information. COP displacement during gameplay was measured during the Vicon version of the Wii Balance board (WBB) while subjects performed various balance tasks [ 33, 68 ]. In this study, wBB COP displacement correlated with the DPSI, a method that uses the COP displacement and measures the time it takes for the user to regain balance during a jump as a measure for postural stability [ 33 ]. Increased COP displacement during gameplay was associated with an increased risk of falling in the elderly [ 25 ]. The COP distance was measured to determine how far the COP movement had been [ 25 ]. This COP distance did not necessarily correlate with the COP distance measured with the WBB [ 25 ]. Also the COP distance during gameplay did not correlate with the DPSI [ 25 ]. In general, COP distance during gameplay did not reflect the DPSI or LOS in elderly [ 25 ]. In young subjects the COP distance during gameplay or the COP distance during standstill did not correlate with the DPSI [ 25 ].
One study on COP displacement in the AP direction [ 25 ] and one on COP distance in the AP direction [ 25 ] were unable to demonstrate an association with any of the DPSI, LOS or gait parameters. The authors concluded that the COP displacement in the AP direction and COP distance in the AP direction during gameplay are more suitable to explain the risk of falling in elderly subjects [ 25 ]. While the COP distance during standstill was not correlated with COP distance during gameplay in elderly, it was highly correlated in young subjects [ 25 ]. In a similar manner a study on the COP distance in the ML direction during gameplay and standstill was not correlated with the COP distance during standstill in healthy elderly subjects [ 25 ]. It was also found that the COP distance during standstill was not correlated with the COP distance during gameplay in older and older-old subjects [ 25 ].
